How much notice is required for medications?

All routine medications require 48 hours notice. Please call and leave a message for the nurse or via the patient portal.

When are appointments required?
Appointments required if not seen in a year. An appointment is required for chronic conditions if not seen in six months.
When is payment due?
Payment is due at the time of service.
What if my insurance changes?
Please update our office with changes to your insurance policy.
What if a service is not covered by my insurance?
In the event your health plan determines a service to be “not covered”, you will be responsible for the complete charge. Payment is due upon receipt of the statement.
What if you don't accept my insurance?

If you have insurance coverage with a plan we do NOT accept, total charges are due at time of service, but speak with practice manager to see if our practice can add your insurance. If we are accepted you may receive a credit.
